The {{documentation subpage}} OEIS Wiki utility template should be used on /doc documentation subpages. It automatically creates a link (see it in action on {{documentation subpage/doc}}, where the top line (in small print) comes from {{documentation subpage}}).
At the start of the /doc subpage, simply use
